Offer 1:

1 Month Trial Plan for Elderly Care:

• Dedicated health manager (A trained and experienced senior nurse)
• 2 home visits by the health manager

• Weekly phone call by Health Manager for follow up

Role of The Health Manager:

During personal visit:

• Overall wellness check (e.g. BP, sugar level, pulse, stethoscope sound etc.). First visit will also include a complete collation of personal & family medical history and current medical status
• Checking doctor’s prescriptions and medicine bought
• Compliance with medicine schedule and dosage
• Compliance of dietary restrictions (if any)
• Make non-prescriptive suggestions for improvement
• Physically accompany the beneficiary for doctor visits/ medical tests etc if needed.
• Based on the status, take action as required (e.g. take appointment with specialist, coordinating for helper/transport for the visit etc.)
• Create report that will be provided online to the customer and in hard copy to the elderly

During phone call:

• Follow up on previous visit
• Follow up on doctor visits (if any)
• Wellness check

Regular coordination:

• Assessment on need for specialist (on monthly visit or via phone)
• Set appointment with specialist, reminder for appointment
• Follow-up post doctor visit
• Coordination of blood tests etc., with home collection of samples and delivery of reports
• In case of health claims, help with paperwork
